Very poor coordination between Pilot and tugs assisting the maneuver.
I think the pilot is not compose at that moment
i think the pilot and tug masters were not qualified for that operation because orders were not being followed or were not properly given o not properly followed given the limited berthing space available.
Always keep parallel to berth till clear as ship was not parallel she was setting to stbd. Then there is deferent current next to berth & away being stronger this gives sudden rotation sheer. So try to be parallel to avoid. Bad correction response of tugs.
